Te-Hsuan Li, Yu-Chang Chan, Chee-Ching Sun
Dermatol Sinica 14: 119-125, 1996

We carried out a study to obtain the epidemiology of erythema multiforme (EM) , Stevens-Johnson syndrome (SJS) , and toxic epidermal necrolysis (TEN) requiring hospitalization and to determine which drug therapies were associated with theses reactions. We reviewed the clinical records of all patients who were hospitalized with these discharge diagnoses at National Taiwan University Hospital from 1985 through 1995. A total of 99 patients were identified in the study; they included 41 cases of EM ..................More
Serum Antibody in Early Stage of Herpes Zoster
Yuh-Chiau Chiang, Yu-Chang Chan, Chi-Cheng Lin, Pei-Gun Lee
Dermatol Sinica 14: 126-129, 1996

In this study, serum VZV antibody was detected in 26 cases of herpes zoster and 13 cases of normal control. The results revealed that the mean titer of the antibody in patient was significantly higher than normal control, although the titers were quite variable in patients individually. Many studies state that active immunization of Oka live attenuated varicella vaccine has the effects of seroconversion or restortion of varicella-zoster virus cell-mediated immune response. They strongly suggeste ..................More

Sindy Hu, Chun-Min Wang, Heng-Leong Chan, Tseng Tong Kuo
Dermatol Sinica 14: 130-135, 1996

Systemic lupus erythematosus (SLE) can associate with various kinds of skin diseases. Among them, papulonodular mucinosis (PNM) is rare by encountered. We report this uncommon association in a native of Taiwan. The patient was a 61-year-old male farmer presented with multiple asymptomatic papulonodules on his upper trunk and bilateral upper arms. A skin biopsy specimen revealed cutaneous mucinosis. He also had SLE confirmed by facial skin biopsy and laboratory tests. Treatment with a combination ..................More
Lupus Erythematosus Profundous with Lipomembranous Change
Tsun-Cheng Wang, Hsiu-Chin Chen, Hsin-Yi Su
Dermatol Sinica 14: 136-142, 1996

A 26-year-old woman had a discoid erythematous patch with underlying subcutaneous nodule on her right arm for 3 years. The histopathological features revealed basal cell hydropic degeneration, patchy lymphocytic aggregates in the mid and lower dermis, and hyaline necrosis of fat tissue intermingled with lipomembranous change. Direct immunofluorescence showed positive lupus band test. Lupus erythematosus profundus with lipomembranous change was diagnosed. We herein report such a rare case and rev ..................More
Neutrophilic Panniculitis in a Diabetic Caused by mycobacterium Cheloinei
Shumin Young, Shyh-Dyi Chuang, Chih-Cheng Chang
Dermatol Sinica 14: 143-148, 1996

An 80-year-old female diabetic had two painful erythematous, indurated subcutaneous lesions on her right buttock, corresponding to previous insulin injection site. The skin lesions soon broke down and formed pus-draining sinuses. Pus smear showed acid-fast positive bacilli. A biopsy specimen was taken from one of the lesions four days after the eruption had appeared and histopathological examination showed neutrophilic panniculitis, Tissue and pus culture yield rapidly growing mycobacteria and M ..................More
Disseminated Fusarium Solani Infection in a Recipient of Bone marrow Transplantation
Lin-Hui Su, Guang-Hsiang Hsiao, Hsien-Ching Chiu
Dermatol Sinica 14: 149-157, 1996

A 21-year-old male patient received allogenic bone marrow transplantation from his HLA-compatible elder brother to treat his aplastic anemia. Fever developed on Day 6 post-BMT. The white blood cell count reached a nadir of 10/ul on Day 8 and, on the same day, some erythematous, edematous, pea-sized papules developed over the face, left thigh, and back. Some of the lesions had central necrosis. A biopsy specimen from the left thigh revealed septate fungal hyphae mainly around and within the throm ..................More
Keratoelastoidosis Marginalis of the Hands with Colloid Degeneration
Jyh-Ren Lin, Guang-Hsiang Hsiao, Hsien-Ching Chiu, Chien-Jui Cheng
Dermatol Sinica 14: 158-165, 1996

We describe a rare association of keratoelastoidosis marginalis of the hands (KEMH) with colloid degeneration. A 63-year-old man has noticed an asymptomatic, rough and uneven skin surface on both hands for one year. Physical examination revealed keratotic, translucent papules or coalesent plaques in linear array on the thenar eminences, the radial aspects of the index fingers and, the ulnar aspects of the thumbs. The clinical picture is typical for KEMH. Histologically, colloid formation as well ..................More
Dariers Disease: A case with Acral Distribution
Fanchiang Jung-Hsiang, Chang Chih-Cheng
Dermatol Sinica 14: 166-170, 1996

We report a case of Darier's disease in a 29-year-old male patient with the presentation of several symmetrically distrbuted hyperkeratotic plaques over bilateral feet for 6 months. In addition to the lesions over the feet, there were several brownish papules over the forehead. Minute pits were also noted over the palms and soles. There was no other cutaneous manifestation. Based on the histological findings of suprabasal clefts and acantholytic dyskeratosis, Darier's disease was diagnosed. The ..................More
Subungual Glomus Tumors A Clinical, Histopathological and Electron Microscopic Study
Kan-Tang Fang, Shoh-Cheng Shih, Jin-Yuan Chen, Gwo-Shin Chen, Hsin-Su Yu
Dermatol Sinica 14: 171-175, 1996

This paper represents the study of two cases of subungual glomus tumor. Extensive investigation conducted clinically, histopathologically and electron-microscopically. The patients suffered from frequent pain especially in cold environment. Histopathological study with Masson's trichrome stain showed dark brown fibers of glomus tumor cells with enlarged circular-shaped nuclear dense stain, situated in the center of the cells. Under the electron-microscopic study, the cytoplasma of the tumor cell ..................More

Dermatol Sinica 14: 176-180, 1996

Only two cases of pseudoxanthoma elasticum (PXE) associated with extensive nuchal comedones and acneiform lesions have been repoted in the English literature. We observed a male patient of PXE, who was also found to have black comedones, reddish inflammatory papules, and acneiform scars on both sides and back of his neck,as well as black comedones on bilateral axillary regions. Along with the two previous cases, all these three patients presenting with characteristic clinical and histological sk ..................More

A 66-year-old female admitted to our ward due to generalized skin eruption and lymphadenopathy for four years. She also experienced nausea, abdominal fullness, palpitation and dry cough. Physical examination revealed generalized erythematous papulonodules with lichenified plaques and excoriated marks and lymphadenopathy. Hemogram showed eosinophilia (absolute eosinophil count 43000/cmm). A 86-year-old man had developed a firm protruding nodule, 3.5cm in diameter, on the left preauricular area for about 3 months. The skin overlying the nodule was brownish to violaceous with dark brownish-red crust coating and scale. Histopathologic examination revealed spindle cells arranged in irregular strands and whorls filling the dermis and extending deeply to the subcutis and musle, some with cartwheel-like arrays around collagenous centers. A 37-year-old male presented a red-brownish, dome-shaped nodule over right shin of 6 months duration. It caused no symptoms and measured 2 x 2 cm. Histopathological examination showed a reverse triangle architecture of intradermal nevus with nests of spindle and epithelioid cell hyperplasia. Giant, bizarre cells and multinucleated giant cells were also revealed. The above histopathological features fitted the diagnosis of Spitz nevus. It is rare for the large size.
