Schnitzler’s syndrome is a rare clinical condition characterized by chronic urticaria, intermittent fever, bone pain, arthralgia or arthritis, and monoclonal immunoglobulin M (IgM) gammopathy. Here we describe a 64-year-old Taiwanese male with a long history of arthralgia, fever, and chronic urticaria with severe pruritus. Bleomycin, derived from Streptomyces verticillus, is used extensively in anti-cancer, anti-viral, anti-bacterial, and anti-tumor therapy. We present herein a case of 50-year-old woman with right pleural effusion due to liver cirrhosis treated with a single intrapleural instillation of bleomycin for cavitary sclerosis. Bowen’s or Paget’s disease involving nipple is not an uncommon event. Surgical excision with nipple-areola reconstruction is usually required for cosmetic reason.
