Buerger's disease or thromboangiitis obliterans(TAO) is characterized by peripheral arterial occlusion in young male cigarette smokers. It rarely occurs in women. We report TAO in a woman who presented with prominent cutaneous thromboangiitis. A 31-year-old female, a heavy smoker, presented with gangrene of toes and many erythematous to brownish nodular and cord-like cutaneous lesions over the lower limbs. The histopathology of both nodular and cord-like lesions showed acute thrombophlebitis in the subcutis and was consistent with the diagnosis of TAO
