Bowel Bypass Syndrome - A Case Report and Literature Review -

A 62-year-old man suffered from sudden onset of painful purplish red papules, patches and some vesicles on a purpuric patch over right hand and wrist with prominent swelling of right little finger and thenar eminence. Histologically, there were accumulation of neutrophils and nuclear dusts in the upper dermis. But no evidence of vascular necrosis or fibrinoid deposit was observed. Due to the past history of receiving Billroth-II gastrojejunostomy surgery 3 months before the appearance of skin eruptions and clinically being well controlled by systemic antibiotics and corticosteroids, our final diagnosis was bowel bypass syndrome.
