We report a case of fatal pancreatitis heralded by the presence of cutaneous panniculitis without abdominal pain or other stigmata of the disease. A 55-year-old male has suffered from high fever, polyarthritis and erythematous subcutaneous nodules on lower extremities, upper extremities and trunk for 10 days. The skin biopsy revealed subcutaneous fat necrosis,"Ghost"cells and polymorphous infiltrates. Serum amylase and lipase were elevated. This patient was dead after 21 days of admission with intensive care.
