Acquired Reactive Perforating Collagenosis

A 42-year-old man with rare acquired reactive perforating collagenosis (RPC) is reported. The man was a victim of NIDDM for more than twenty years associated with triopathy and end stage of renal disease under regular insulin injection and hemodialysis. Multiple discrete hyperkeratotic and pruritic papules with umbilicated plugs scattering on trunk, upper and lower extremities, and scalp area developed for more than two years. In the skin biopsy specimen, a typical hyperkeratotic papule with a central plug showed uleceration, surrounding epidermal hyperplasia, and eosinophilic vertical strands extending from the dermis through the epidermis. Masson stain proved the perforating strands to be collagen fibers.
