Evaluation of the Result of Surgical Treatment

Basal cell carcinoma is a kind of malignant skin tumor. Surgery is one of the methods of treatment, but recurrence of the tumor is a troublesome problem to doctors. During the 10 years between 1981 and 1990 in Changhua Christian Hospital, 45 cases of primary and solitary basal cell carcinoma were noted. After surgical treatment, they had been followed up for about 6 months to 11 years. The surgical methods in our cases include simple closures, local rotation flaps and skin grafts after wide excisions with margins of 5 mm in width and with depth down to the fascia. After the follow up of average 5.4 years, 2 cases were found to recur and the recurrence rate was 44% Through the experience of these 45 cases and the review of literature, we think that it is feasible to excise the primary basal cell carcinoma with a 5 mm margin But when the tumor is located in the orbital or nasal area where sun exposure is more easily, and the histopathologic finding reveals infiltrative ytpe, it is better to excise the tumor with a margin more than 5 mm in width or to perform Mohs' surgery.
