In this study we compared the efficiency of chemotherapy between dapsone monotherapy and multidrug therapy in multibacillary leprosy outpatients.Thirty-four cases received DDS monotherapy and 38 cases received multidrug therapy. The annual mean bacterial index fell within the first three years were 0.94±0.40, 0.69±0.31 and 0.59±0.30 in the DDS group, in the multidrug therapy group they were 1.38±0.52, 0.94±0.54 and 0.86±0.56 respectively. The difference between these two groups were all significant (p<0.05) in the first three years.Erythema nodosum leprosum was less common (8 cases, 21.1%) in those treated with multidrug therapy than in those treated with DDS monotherapy (16 cases, 47.1%) during the treatment period. 12 cases (35.3%, 3.94 per 100 patient-years) relapse were observed in the DDS monotherapy group and only 1 case relapse (2.7%, 0.55 per 100 patient-years) was observed in the multidrug therapy group. From our study we are led to the conclusion that efficiency of chemotherapy with multidrug therapy was much better than those with DDS monotherapy in the therapy of multibacillary leprosy patients.
