Coexistent Bullous pemphigoid and Systemic Lupus Erythematosus: The Diagnostic Dilemma

A vesiculobullous eruption with clinical and histological features of bullous pemphigoid (BP) developed in a 74-year-old housewife with later proved systemic lupus erythematosus (SLE). The diagnosis of BP was based on indirect immunofluorescence studies on 1 M NaCl-split normal human skin showing high titer (1:320) IgG deposition along epidermal roof. This finding also discriminates between BP associated with SLE and other subepidermal bullous diseases, including bullous SLE, epidermolysis bullosa acquisita and dermatitis herpetiformis.
