Tuberculosis Verrucosa Cutis Associated with Cutaneous Ulcer

A 39-year-old male patient presented with an exacerbated ulceration on his left intergluteal to inguinal area for two months. During his childhood, the lesion was a small nodule on his left buttock. With age, it slowly enlarged into an irregular verrucous plaque with itching sensation and occasional erosion due to over scratch. Wound culture yielded Pseudomonas aeruginosa. Histopathological examination revealed a granulomatous infla-mmation with caseous necrosis and multiple Langhan's giant cells. Scanty acid fast bacilli were found by Ziehl-Neelsen stain. It complied with the diagnosis of tuberculosis verrucosa cutis with secondary bacterial infection. After one week treatment with anti-TB drugs and antibiotic(ciprofloxacin), the ulcerative wound improved remarkably and nearly healed one more week later.
