Papular Elastolytic Granuloma-----A Variant or an Early Stage of Annular Elastolytic Granuloma

Annular elastolytic granuloma (AEG) is a rare form of dermatosis. Although it usually manifests as annular lesions clinically as reported in the literature, papular lesions with coexisting annular lesions have also been mentioned in some reports. We describe a 53-year-old woman who presented with several grouped bean-sized reddish-brown papules on both sides of her neck, which had grown insidiously for 6 months. Histopathologic examination of two lesions revealed typical features of AEG with fiber-engulfing giant cell granulomas, absence of elastotic and elastic fibers inside the granulomas, and prominent actinic elastosis peripheral to the granulomas. Based on the clinicopathologic findings, we consider them to be a variation or an early papular manifestation of AEG.
