Bacille Calmette-Guerin (BCG) vaccine is administered to prevent tuberculosis and is considered to have an excellent safety profile. However, severe BCG infection can occur in individuals with defects of the host immune response. We report an eight-month-old male baby suffered from severe combined immunodeficiency with disseminated BCG infection. Skin biopsy showed numerous acid-fast bacilli and PCR analysis of skin, cerebral spinal fluid and gastric aspirate all identified Mycobacterium bovis. Patient died despite anti-TB drugs and bone marrow transplantation.
