There are many methods to treat keratoacanthoma. Surgical excision is the treatment of choice for majority of keratoacanthoma. However, significant cosmetic defects can result if large or cosmetically important location are involved. Intralesional therapy with 5-fluorouracil, bleomycin, methotrexate or interferon-α has been shown to be effective in treatment in these cases. We report a case of keratoac-anthoma on nasal ala with extension to nasal cavity resolved in 3 weeks after treated with intralesional methotrexate after 2 injections. We concluded that intralesional methotrexate is a simple and effective treatment for solitary giant keratoacanthoma. It offers greater efficacy, more rapid course, decreased pain and lower cost compared with other intralesional agents and deserves recommendation.
