Speckled lentiginous nevus is characterized by a tan or light brown background patch on which more darkly pigmented macules or papules are randomly distributed. Histologic examination of the tan patch reveals features of lentigo simplex, whereas the speckled macules and papules disclose melanocytic nevi. We report a case of a 6-year-old boy with multiple large congenital lesions on the head and neck. It is an unusual clinical presentation of speckled lentiginous nevus which must be differentiated from benign pigmented lesions due to a potential for malignant changes.
