Merkel cell carcinoma is a rare primary neuroendocrine skin tumor that usually arises in the head and neck of elderly patients. Local recurrence and metastasis are common and the mortality rate has been reported to be as high as 30%. There is a high incidence(25-34%) of coexistence with second neoplasm, including other skin tumors and hematological malignancies in patients with Merkel cell carcinoma. However, mixed Merkel cell and squamous cell carcinoma in the same lesion is very rare. Herein we report a 89-year-old women with such a tumor. According to the previous studies, the local recurrence rate of the mixed tumor is much higher than that of Merkel cell carcinoma occurred alone. Regretfully, patient refused complete excision and was lost to the follow-up.
